Silver Springs Care Center provides short-term rehabilitation in Meriden, Connecticut for individuals recovering from hospitalization, surgery, illness or injury. Through our Touchpoints Therapy program, residents have access to physical, occupational and speech therapy as part of an individualized rehabilitation plan focused on restoring function, building independence and preparing for the next step in recovery.

Our rehabilitation and nursing teams work together from admission through discharge, developing goals around each resident’s abilities, needs and plans for returning home or transitioning to the most appropriate level of care.

Rehabilitation Designed Around the Individual

Every recovery is different. Some residents need to rebuild strength and endurance after a hospitalization. Others may need to regain mobility following surgery, improve their ability to complete everyday activities or address communication and swallowing difficulties.

Our rehabilitation team evaluates each resident and develops an individualized treatment plan based on their specific needs and recovery goals.

Therapy services are available up to seven days per week and include physical therapy, occupational therapy and speech therapy.

Physical Therapy

Physical therapy focuses on improving the strength, mobility, balance and endurance needed to move safely and independently.

Depending on individual needs, physical therapy may include:

  • Walking and ambulation training
  • Strength and endurance training
  • Balance and fall-prevention strategies
  • Transfer training
  • Mobility and positioning
  • Assistive device training
  • Stair and mobility preparation for returning home

The goal is to help each resident achieve the highest practical level of mobility and independence.

Occupational Therapy

Occupational therapy helps residents regain or adapt the skills needed to complete everyday activities as safely and independently as possible.

Therapy may address:

  • Activities of daily living, including dressing, bathing and grooming
  • Adaptive equipment education and training
  • Upper-body strength and coordination
  • Functional mobility
  • Energy conservation techniques
  • Cognitive skills related to daily activities
  • Home safety and discharge preparation

Speech Therapy

Speech-language pathology services can address communication, cognition and swallowing difficulties associated with illness, injury or other medical conditions.

Depending on the resident’s needs, speech therapy may focus on:

  • Memory and problem-solving strategies
  • Swallowing difficulties
  • Strategies to support safe eating and drinking
  • Patient and family education

Skilled Nursing and Rehabilitation Working Together

Successful rehabilitation often involves more than therapy alone.

Residents recovering from hospitalization may also require skilled nursing for medication management, wound care, chronic disease management or other clinical needs. At Silver Springs, nursing, rehabilitation and other members of the interdisciplinary care team work together to coordinate these services.

This collaborative approach allows the team to consider the resident’s overall health and functional abilities while working toward rehabilitation and discharge goals.

Preparing for a Safe Return Home

Discharge planning begins early in the rehabilitation stay.

Our rehabilitation, nursing and social services teams work with residents and families to identify what will be needed for a successful transition from Silver Springs.

Depending on individual circumstances, preparation may include:

  • Activities of daily living retraining
  • Mobility and assistive equipment training
  • Home safety preparation
  • Patient and caregiver education
  • Medication and care coordination
  • Recommendations for equipment or services
  • Coordination of home health or outpatient services when appropriate
  • Planning for transition to home or another level of care
